If you require any vaccinations relating to foreign travel you need to complete a Travel Questionnaire and return it to the surgery as soon as possible.  The Practice Nurses will then check the latest travel advice and find out which vaccinations (if any) are required.  You will need to contact the surgery a few days after you return your questionnaire to find out if you need an appointment.  Ideally you need to start any vaccinations 6-8 weeks before travel.  Not all vaccines are kept in stock and some will need to be ordered.  Most travel vaccines are free on the NHS but there are some more specialised vaccines which will incur a charge which must be paid in full beforehand.

Some travel vaccines are ordered on a private prescription and these incur a charge over and above the normal prescription charge. This is because not all travel vaccinations are included in the services provided by the NHS.

Travel Health Questionnaire

Please complete a Travel Questionnaire which is available at reception.  You will need to complete one questionnaire for each registered patient travelling (including children).  Please note that if we do not get enough notice before you travel we may not be able to give you an appointment so you will therefore be directed to a local travel clinic.
